Registration Requirements:
1. Birth Certificate
2. Immunization Record
3. Social Security Number
4. Proof of Residency (electric bill, rental agreement, etc.)
5. New Student Enrollment Form
6. Annual School Health Record
7. McKinney-Vento Residency Information Form

Note: Forms can be printed and filled out ahead of time or provided by the school office.
